Hey guys, its me again with my obsession for achievements.
Here is the list for all the 11 new achievements available with the DLC and while i have no idea of the specific requirements for each one, i will take quite the amount of time to reach and test them myself since i like to do things in my own time. And because of work... Mostly work.
All seem to be straight forward, not requiring core difficulty but any help is welcome to confirm and compile them to the guide for all achievements.

Pretty sure Wings of Stone requires you to go to the library in Chapter 1 after getting the DLC.
Beware Still Waters can be gotten during Ulbrig's first quest. I think I just had to pass an athletics check.
I believe Elements of Success is also in the first quest. You'll come upon some non-hostile elementals. There is an enemy nearby that can aggro them. I attacked the first elemental I saw, so no idea if there are additional steps.
I think This Wall Won't Fall, Not the Last Sarkorians, and The Lady of the Forest are all plot related, but I'm still working through.

To clarify Sellout as rereading it I think I left out some important details, during Ulbrig's second quest there are several side quests you can do in the new area you go to. One of them is to investigate the sickness for the chieftain and that quest is the one with the opportunity for Sellout.
I'll have another few in a couple of minutes as soon as I confirm that I got one of them (load times are killing me right now).

Elements of Success is correct, I believe you have to interact with an item but it’s a very prominent one

Another achievement, I got Sellout during a side quest during Ulbrig's second quest. You can either collect two out of the three(?) pieces of evidence (note found on an enemy surrounded by bodyguard golems, talk to the quarantined people, and talk to the guy south of Ysenna who mentions his dog).
Or just brute force it with the Aeon option. If only every detective had an Aeon on call.
You get the achievement after the fight that follows.

Alright, one of the tools I use for datamining got updated, so I can fill in some info on some of the achievements I missed.
Endangered Species - It is romance specific. The trigger condition is for UlbrigRomanceFindTraces_c3 to be equal to 4, so based on the variable name I am assuming it is his chapter 3 romance event.
Stay Underground - To find the Worm, you need to remove some planks from a cave entrance. If I am interpreting the files correctly, the cave is in the main Gundrun map, but I don't know where. The achievement unlocks after you burn the worm's corpse.
This Wall Won't Fall - Looking at the conditions (killing a specific unit), I don't think it is possible to fail, which I was uncertain about before.

The cave is in the southeast part of the map. There are a couple of athletics/mobility checks to get to it where you have to climb/descend. Once you get into the cave, there are a few smaller fights with devoured guards + their swarms then there are planks (athletics DC 29) to remove and the worm is behind them. Killing the worm gives stay underground.

I've been at this a while, so I know what to look for in the game files. Also I have a lot of spare time. :P
Oh and looking at the files while I am compiling stuff for my guides, I think Stay Underground might be tied to the Ulbrig Quest 2 side quest "Whence Moves the Hunger?" While playing, I thought I completed that quest, but I guess I didn't which would be why I missed it.

Hanged Man Notes (During Quest 2)
Page 117 - Knock down the corpse (athletics) near the Spirit Catcher
Page 11 - Perception check by three ghosts chatting (near 117)
Page 23 - In front of Forest Rangers cabin (bottom right of mini-map) near devoured bear
Page 48 - Perception check right before you encounter three spirit wolves fighting demons, near an entrance to the abandoned cave
Sarkoris Will be Green
In the Forest of Forgotten Spirits there are two caves (Swamp Cave and Abandoned Cave), inside the two are two unique enemies. Each carries a key.
Take the two keys to a door in the wall near the start of the Forest. You need to insert these keys, but only one half is usable at a time. So after inserting the first skip a few hours until the other half is available to use. Once you put both in, you can enter the Cave of Lamentations. Kill the enemies to get the trophy.

So some final notes after having beaten the DLC.
- This Wall Won't Fall - Part of Ulbrig's third quest. Not sure if it is guaranteed or if there is a chance of failure
- Not the Last Sakorian - Complete Ulbrig's third quest
- Stay Underground - I did not encounter the lair. I assume you have to do this during Ulbrig's second quest as the giant worm attacks earlyish in the third quest if you didn't kill it.
- Endangered Species - I did not get this one. It is possible that it is somewhere near the giant worm lair and I just missed the area. Alternatively, it could be romance related or a time limited event (e.g., I loaded a Chapter 5 save so if it required Chapter 2 I would have missed it).
